The Community Case Manager for the Elderly is available to assist any Norwalk resident who is 60 years of age or older.

  • Home visits or office appointments available
  • No fee for services
  • No income restrictions
  • Donations are accepted

The Case Manager provides:

  • Outreach
  • Advocacy
  • Senior Housing Options
  • Health Insurance Counseling
  • Information & Assistance
  • Short Term Counseling

Assists in applying for the following benefits and services:

  • Energy Assistance-Money for heating costs
  • ConnPACE-Helps pay for medications
  • Medicaid-State medical plan
  • Rent Rebate
  • Homeowner's Tax Credit
  • Lifeline-Emergency response service
